- Information you send us: your name, phone number, optional email address and the procedure you are interested in, when you submit a consultation form.
- Usage information: pages visited, approximate location, device and browser type, collected through Google Analytics using cookies.
Please do not submit health records or photographs through this website. Clinical information is collected privately at your consultation.
How we use it
- To contact you about the consultation you requested
- To arrange appointments with our partner surgeons and clinic
- To understand how the website is used and improve it
We do not sell your personal information.
Service providers
Form submissions are processed by Splitforms, and website analytics by Google Analytics. These providers may store information outside Canada, including in the United States, where it may be subject to local law. They process information only to provide their services to us.
